Referral and Intake Meeting

At this meeting, the parent(s) and the child meet with a team of professionals (assessment team) from the EC PreK program. This team may consist of an LEA representative, a special and regular education preschool teacher(s), a school psychologist, a speech pathologist, an occupational therapist, and a physical therapist.  Referral paperwork will be completed which includes the strengths and needs of the child and the reason for the referral. Team members may conduct observations and possible screenings as well as review any previous evaluation results to decide if further evaluations are needed to determine eligibility. Evaluation information may be required in five areas of development in order to determine eligibility: cognitive, social, motor, adaptive, and communication. The assessment team must also have a hearing and vision screening that has been passed within one year of the referral.  Hearing and vision screenings may be conducted during this meeting.  If the team decides that more testing information is needed, a decision will be made as to what type of assessment is necessary and how it will be conducted (traditional testing or play-based assessment).  Appointments will be scheduled with parents to complete any recommended testing.  If the team decides no additional testing is needed, the team will either complete the final work at that time or schedule an eligibility meeting prior to the child’s third birthday.  The child is required to attend this meeting, which will last about 1 hour.  A copy of the child’s birth certificate and immunization records is required.  Parents must also provide 2 proofs of residence at the initial referral meeting.  This documentation must show the parent’s name and current home address (driver’s license, utility bill, rental agreement, etc.).

Multidisciplinary Assessments for Preschoolers

Multidisciplinary teams typically consist of a preschool teacher, a speech-language pathologist, a psychologist, and, when needed, an occupational therapist and/or physical therapist.  Parents will be asked to fill out background medical and developmental information on their child as well as provide information on adaptive and social skills. There are two different types of assessments that can be conducted with preschoolers.  One type is a “traditional” or “standardized” assessment where one of the assessment team professionals will work with the child in a structured setting. The other type of assessment is called a “play-based” assessment or Transdisciplinary Play-Based Assessment (TPBA).  This is conducted by the assessment team by observing the child playing in an environment that is set up like a small preschool classroom.  There will be a play facilitator who will encourage and elicit certain play and pre-academic skills from the child while the observers record and interpret information.  Assessments typically take about 1.5 hours with the parents present.  The child is required to attend all testing appointments.

Eligibility Meeting

After assessments/evaluations are completed, an eligibility meeting will be scheduled to discuss the results and determine eligibility for services and possible placement opportunities for intervention services.  If a child is eligible for services with the EC PreK program, an Individualized Education Plan (IEP) will be developed.  The type and frequency of intervention services recommended are dependent upon the goals developed on the IEP and the needs of the child. This meeting will last 1-2 hours.  The child is not required to attend this meeting.

Referencia/ Primera Reunión

En esta reunión, los padre(s) y el niño se reúnen con  un grupo de profesionales (equipo de evaluación)  del Programa de Preescolar EC. El equipo está formado por una representante del sistema escolar LEA, una maestra (s) de educación especial y regular, psicólogo escolar, patóloga del habla,  terapista ocupacional, terapista física. Los  documentos de referencia serán completados y se debe incluir información de áreas fuertes y de necesidad del niño y razón para referencia. Los miembros del equipo quizás realicen observaciones y posiblemente harán pruebas así mismo como la revisión de resultados de evaluaciones previas para decidir si futuras evaluaciones son necesarias para determinar elegibilidad. Información de evaluaciones es requerida en cinco áreas para determinar elegibilidad tales como cognitiva, social, motriz, adaptación y comunicación. El equipo de evaluaciones también necesita pruebas auditivas y de visión que hayan sido realizadas dentro del año de referencia. Pruebas de visión y audición quizás sean completadas durante esta reunión. Si el equipo  decide que se debe obtener más  información a través de evaluaciones, una decisión será tomada determinando que tipo de evaluación es necesaria y como será conducida (evaluación tradicional o evaluación a base de juego). Si el equipo decide que no hay necesidad de más evaluaciones, el equipo decidirá completar la documentación  final en este momento o hacer una cita para la reunión de elegibilidad antes del tercer cumpleaños del niño. Se requiere que el  niño asista a esta reunión la cual tiene una duración de aproximadamente 1 hora.   Una copia del acta de nacimiento y registro de vacunas son requeridas. Padres también deben proveer prueba de residencia en la primera cita de referencia. Esta documentación debe mostrar el nombre del padre y la dirección actual de la casa (licencia de manejo, cualquier factura de servicios públicos, contrato de arrendamiento, etc.)  

Evaluaciones Multidisciplinarias para Preescolares

El equipo multidisciplinario consiste típicamente de una maestra de preescolar, patóloga del habla/lenguaje, psicólogo, y cuando es necesario una terapista ocupacional y/o  una terapista física. Se les pedirá a los padres que llenen documentación de historia médica e información de desarrollo de su hijo así mismo como proveer información en las habilidades sociales y de adaptación. Hay dos tipos diferentes de evaluaciones que se conducen con preescolares. Uno de ellos es llamado “tradicional” o “Estandarizado” en la cual uno profesionales del equipo de evaluaciones trabajara con su hijo en un entorno estructurado. El otro tipo de evaluación es llamado “Evaluación a base de juego”  o Evaluación Transdisciplinaria a Base de Juego (TPBA). Esto  es conducido por el equipo de evaluaciones  quienes  realizan  observación del niño mientras juega en un ambiente algo familiar el cual está arreglado como “una mini clase de preescolar” Habrá un facilitador de juegos quien incentivara e iniciara ciertos juegos y habilidades pre académicas con el niño mientras los observadores registran e interpretan información. Evaluaciones usualmente toman de 1.5 hora con los padres presentes. Es requerido que el niño asista a todas las reuniones de evaluaciones.

Reunión de Elegibilidad

Después  que las evaluaciones son completadas, una reunión de elegibilidad será citada para discutir los resultados, determinar elegibilidad para servicios y posibles oportunidades de ubicación para servicios de intervención. Si el niño es elegible para servicios con el programa de Pre-K EC,  un IEP (Plan Educativo Individual) será desarrollado. El tipo y frecuencia de servicios de intervención recomendados dependerán de los objetivos desarrollados en el IEP y las necesidades del niño. La reunión durara aproximadamente entre  1 a 2 horas. No es requerido que el niño asista a esta reunión.

Proceso de Referencia y Evaluación

•  If you are a parent, physician, or childcare teacher and have concerns about the speech and/or overall development of a preschool-aged child and would like to make a referral for an exceptional children’s evaluation, please call Caroline Price at 704-282-6259 ext. 0.  You will need to provide information on the child’s physical, motor, educational, communication, behavioral, and daily living skills in a phone interview.  An observation and/or referral meeting will be scheduled shortly after the phone interview. 
• If you are a parent of a preschool-aged child with an IEP that has recently moved into Union County, please call Caroline Price at 704-282-6259 ext. 0 to schedule a meeting to determine comparable services and review the IEP.  You will need to provide the preschool assessment center with a copy of your child’s IEP prior to the meeting.

• Si usted requiere de un traductora de español y tiene preocupaciones acerca del desarrollo del lenguaje/habla o del desarrollo en general de un niño de edad  preescolar y le gustaría hacer una referencia para una evaluación con el Programa de Niños Excepcionales por llamar a Iris Crispin al 704-282-6259 ext. 2. Usted necesitara proveer información de las habilidades físicas, motrices, educacionales, comunicación, comportamiento y  de diario vivir a través de una entrevista telefónica. Una observación y/ o reunión de referencia será citada en un corto tiempo después de la entrevista telefónica.
• Si ustedes necesitan una traductora de español y son padres de un niño de edad preescolar quien tiene un IEP y recientemente se han movido al Condado de Unión, por favor llamar a Iris Crispin al 704-282-6259 ext. 2 para hacer cita para una reunión para determinar servicios comparables y revisión del IEP. Usted debe proveer al Centro de Evaluaciones de Preescolar con una copia del IEP de su hijo antes de la reunión.
